I GIVE, DEVISE, AND BEQUEATH to New York University Langone Hospitals (NYULH), having its principal office at 550 First Avenue, HCC15, New York, NY 10016, to be used for [Purpose] (or by) the Department of ____________________________, [the sum of $__________ and/or ______% of my residuary estate. If the circumstances at NYU Langone Health should change rendering the purpose of this gift impracticable or impossible, then I intend that this gift be used for such purposes that are most in keeping with the purpose described herein as recommended by the dean and CEO of NYU Langone Health.
